PERSONALIZED TREATMENT PLANS
We focus on individualizing each patient’s care…there is no one-size-fits-all care. We incorporate multiple evidence-based lifestyle change modifications specific to your particular medical needs.
Your journey will begin with a thorough medical history, physical examination and lifestyle assessment.
At Lifestyle Medicine Transformation, we will work together with you and your physician(s) to possibly avoid, decrease, or stop all prescription medications involved in your care as demonstrated by physical examination, laboratory testing and your body’s response to lifestyle medicine interventions.
We implement evidence-based lifestyle medicine modalities to help improve the quality of your life and longevity.
COLLABORATIVE CARE
At Lifestyle Medicine Transformation, we will work in collaboration with your primary care physician and/or specialist physicians(s) from an evidence-based lifestyle medicine perspective to further improve your overall health and well-being.
We have 3 specific goals… 1) to work with you to further incorporate healthy lifestyle modalities, 2) to work with you to become a more active participant in your healthcare management, and 3) to work with you and your physician(s) to possibly avoid, decrease, or stop all prescription medications involved in your care as evidenced by physical examination, laboratory testing and your body’s response to lifestyle medicine interventions.
NUTRITION
Working with you to obtain or maintain your nutritional health is the cornerstone of what we do at Lifestyle Medicine Transformation.
We do this by placing you on a predominantly plant-based diet, with special attention to your particular medical and health needs.
As Lifestyle Medicine physicians, we specialize in the management of preventing, treating, improving or potentially reversing all types of chronic disease states. These diseases may include high blood pressure, elevated cholesterol levels, Type 2 diabetes, heart disease, obesity, gastrointestinal issues, autoimmune conditions, cancer as well as many other medical conditions.

EXERCISE
According to the World Health Organization, physical inactivity or “deconditioning” is the fourth leading risk factor for global mortality.
Approximately half of all Americans do not meet the U.S. guidelines for aerobic physical activity.
At Lifestyle Medicine Transformation, we believe that exercise should be included as a standard component of your medical management. We consider exercise to be another form of medicine.
Our approach is to work with you to develop an individualized exercise “prescription” program that emphasizes the health benefits of physical activity on the body and mind. Many chronic diseases such as diabetes, hypertension, obesity and high cholesterol can be significantly improved with appropriate exercise training.
SLEEP
MANAGEMENT
Sleep truly matters. Approximately one third of our life is normally spent sleeping. Between 50-70 million Americans suffer from sleep deprivation. Sleep is essential for restoration and renewal of the body. A large proportion of DNA remodeling and other cellular and biochemical processes advantageous to one’s health occur during the hours of sleep. The lack of sleep is known to worsen some chronic medical conditions such as obesity, high blood pressure and diabetes.
At Lifestyle Medicine Transformation, we will work with you to assess your possible barriers to restorative sleep, help you develop a plan for better sleep, and maximize your sleep management goals

REDUCTION
Seventy percent of primary care doctor visits are related to stress and the lack of emotional well-being. Over 40 million Americans suffer from some type of anxiety disorder. There is a direct relationship between one’s mental health and many chronic diseases such as high blood pressure, obesity, diabetes and cancer.
Your mental health is important at every stage of life and includes your emotional, psychological, and social well-being. Additionally, your mental health determines how well you handle stress, how you relate to others, and how you make life choices.
At Lifestyle Medicine Transformation, some of our wellness goals are to ensure that you are enjoying life, to assist you with coping skills, to ensure that you are able to set and fulfill personal goals, and to have you build and maintain healthy relationships with others.
